Your first visit to Lemont Chiropractic Center establishes a vital foundation for our relationship with you. For many people this may be your first time seeing a chiropractor, and most are a little anxious or nervous, so after we obtain your important background information and medical history, we give you time to get to know your chiropractor,and take the time to explain what to expect regarding the initial examination and treatment. We do not begin until you are fully aware of what to expect, and are comfortable enough to get started. We want your first visit to be as relaxed and stress free as possible.
To save some time you can either print out the patient forms from this website (see below), or we can email them to you, so you can leisurely fill them out before your appointment. We make every attempt to see you the day you call; however, because a new patient visit requires more time, the days schedule may not allow it. We will try our best to see you as soon as possible.
